Patient Forms:
We want to make things easier for you and we know your time is valuable. If there are any intake or consent forms needed to be filled out, our friendly staff will send you an email to complete these forms online. If you are unable to complete these forms, please arrive at least 15 minutes prior to your appointment.
On Time Arrival:
We ask that you please arrive on time for your appointment, which means allowing time for traffic and parking. For new consultations, we ask that you arrive 10-15 minutes prior to the start of your appointment to fill out intake paperwork.
Late to Appointment Policy
Your time is valuable, so we try our best to ensure appointments run on schedule. We pride ourselves in giving each and every patient the attention they deserve. As a courtesy to all of our patients, if any patient is late for more than half of their appointment time, we will need to reschedule to avoid inconveniencing patients scheduled after them. Patient will then forfeit their deposit. No exceptions or extensions.
Cancellation Policy:
We have a 24-hour cancellation policy to protect the valuable time of our providers. If you need to cancel your appointment for any reason, we ask that you provide a 24-hour courtesy notice otherwise your deposit will be forfeited for the provider’s lost time. Considering our 24-hour cancellation policy, when booking appointments, we require a deposit to secure your appointment. Our consultations require a $50 deposit and our treatments require a $150 deposit. For any appointment cancelled less than 24-hours before the start time, the deposit will be forfeited. No exceptions or extensions. If this should happen more than once the practice may at it’s discretion choose to discontinue your care.
Missed Appointment “No Show” Policy:
While we make every effort to provide a reminder call at least 24 hours before your appointment, it is your responsibility to remember your appointment. If there is a missed appointment the deposit will be forfeited. No exceptions or extensions. If this should happen more than once the practice may at it’s discretion choose to discontinue your care.
